Job Description:
Conduct interview to all seeking admission/treatment to determine eligibility for MSS assistance.
Conduct regular ward visits/rounds to the patient.
Conduct collateral interview through home visitation and follow-up to abandoned and referred cases.
Refer and conducts abandoned patients, pickup, homeless and mentally-ill patients to other agencies for institutionalization.
Attend to patients medical needs as well as complaints.
Provide outright counselling with patients and relatives before discharge of patients in the hospital.
Prepare referrals/letters and other communication necessary for networking, linkages and coordination with community resources.
Record progress notes/reports regularly for abandoned and referred patients.
Perform other related functions as assigned by the immediate supervisor.
Qualifications:
Must be a Registered Social Worker
