Rate: $4/hr, and earn up 40,000 PHP +/month.

Responsibilities: 
Conduct telephonic follow-up with patients and/or caregivers, identifying clinical and social needs and/or gaps in care
Partner with social and clinical colleagues, PCP, and hospital-based clinical teams to help coordinate internal and external resources needed to manage the patient's plan of care
Perform triage assessments, escalating acute issues to Advance Care Provider team
Assist with scheduling PCP/specialist follow-up
Respond to patient and/or caregiver phone calls
Provide education to patients and caregivers
Conduct clinical intake visits with prospective patients via video and/or phone. Intakes include verifying and documenting a clinical history, verifying medications, answering basic questions about services, and identifying and flagging concerning history or labs for provider review.
Educate patients about their journey with us, setting them up for clinical success.
Monitoring and assigning provider support requests and initiatives.
Providing updates to clinical intake workflow
Delivery of just in time feedback as needed
 
Good to Have Qualifications:
California-licensed Registered Nurses
Telehealth or phone triage experience is a HUGE PLUS (but not required)
Experience with CareXM
