HOME HEALTH SCHEDULER (WORK FROM HOME)
 
The Scheduler plays a vital role in coordinating patient visits, maintaining accurate schedules, and ensuring smooth communication between field clinicians, office staff, and referral sources
 
Qualifications:
Must have Scheduling experience in Home Health using Kinnser
Responsibilities:
Plot visits in Kinnser
Review Frequency and Duration in Kinnser
Contact clinicians for their schedule and frequency and duration 
Check CareStitch notes
Coordinate and manage schedules for field staff, ensuring patients are seen promptly and according to their care plan
Maintain up-to-date scheduling records, logs, and client rosters
Communicate scheduling changes promptly to field and office staff
Verify visit frequency and compliance after notes are matched to billing records, reporting discrepancies to the supervisor immediately
Prepare and distribute weekly field staff schedules
Assist with coordination of patient services and relaying messages among clinicians, office staff, and liaisons
Support referral intake processes by preparing documentation and distributing copies to appropriate team members
Assist with processing therapy notes, obtaining visit counts, and reporting weekly summaries to the Director of Nursing
