About the Role
 
The Home Health Clinical Intake Coordinator Nurse is responsible for coordinating the clinical intake process for new home health referrals, ensuring timely admissions, accurate documentation, insurance verification, and compliance with Medicare, California regulations, and agency policies. This role works closely with referral sources, physicians, clinical staff, and the billing department to facilitate efficient patient onboarding and continuity of care.
 
Qualifications
A Bachelor's degree in Nursing and an active Philippine Registered Nurse license is a must have.
Prior experience in home health intake, clinical coordination, or home health operations.
Knowledge of Medicare home health eligibility requirements, homebound criteria, face-to-face requirements, and physician orders.
Familiarity with insurance verification, authorization processes, and managed care requirements.
Experience with home health EMR systems; WellSky/Kinnser required.
Strong organizational, communication, documentation, and problem-solving skills.
Ability to manage multiple referrals and meet time-sensitive admission deadlines.
Clinical background as an RN required.
Key Responsibilities
Review incoming referrals for clinical appropriateness, completeness, and eligibility for home health services.
Verify insurance coverage, authorization requirements, and payer-specific admission criteria.
Coordinate with hospitals, physicians, case managers, and referral sources to obtain orders, clinical records, and missing documentation.
Review patient diagnoses, face-to-face documentation, medication lists, and supporting clinical records.
Coordinate start-of-care (SOC) scheduling and communicate admission details to nursing, therapy, and other clinical disciplines.
Ensure timely admissions, including coordination of SOC visits within 24 hours when appropriate and ordered.
Identify clinical concerns, referral gaps, and potential barriers to admission; escalate issues to the Clinical Manager as needed.
Ensure intake documentation is accurate, complete, and entered into the agency's EMR system Kinnser.
Track referrals, admission status, pending authorizations, declined referrals, and reasons for non-admission.
