The Home Health Scribe is responsible for remote documentation of comprehensive assessments performed by Registered Nurses in the home setting.
The Home Health Scribe is responsible for accurately capturing the assessment findings of the Registered Nurse, accurately scoring the OASIS and developing the comprehensive Plan of Care.
The Home Health Scribe is responsible for transcribing real-time assessment findings and medical information from patient encounters into the electronic medical record, collaborating with the assessing clinician to ensure accurate transcription and completion of all documentation same-day.
Duties and Responsibilities: * Ensures medical record compliance by self-documentation attestation.
Prepares, transcribes, and assembles medical record documentation/charts for physician(s)
Updates patient history, physical exam, and pertinent health information of the patient
Complies with hospital and medical facility policies, including those relating to HIPAA and Joint Commission.
Prepares and sends all documentation to physician for review and approval
Analyze the patient history and medical records before input of data to the assessment record.
communicate with clinician/nurses to ensure that assessment form is filled up correctly with the appropriate information.
Uses a client system tool to record all data that was scribe within the session with the clinician/nurses
Performs other duties and tasks to improve productivity and workflow as assigned.

Requirements:
Registered Philippine Nurse
At Least 1 year of clinical experience
Excellent communication skills
Work experience with OASIS is an advantage
Detail-oriented
Strong written, oral and interpersonal communication skills
Willing to work on night shift
