Job Summary:
 
The PTI Social Worker is primarily responsible for the case management tasks with clients assigned to them. PTI Social Worker offers guidance and support for the client to achieve economic empowerment by providing case management, career counseling, employment preparation and referral, and crisis intervention. Critical to the role is the ability to develop a trusting relationship with clients by adapting principles of motivational interviewing, utilize strengths-based approaches in developing the client's self-efficacy, and adhere to ethical and professional standards.
 
Duties and Responsibilities:
 
Case management:
Facilitate client intake procedures and assess client needs 
Develop, implement, monitor, and evaluate client case plans and progress
Facilitate case coordination and information sharing with client social workers and other service providers involved
Process client referrals and applications to PTI sub-programs (education, work immersion, social services, entrepreneurship)
Coordinate with assigned PTI sub-program staff (education, work immersion, social services, entrepreneurship) 
Monitor client's progress and determine case closure following standard protocols
Maintain client records and progress notes
Career counseling:
Facilitate client goal development activities to determine their plans and action steps
Assess motivational and psychosocial issues
Conduct counseling sessions to determine client's challenges and help facilitate identifying solutions
Conduct sessions using motivational interviewing with inactive clients to identify plans and determine intervention
Employment preparation and referral:
Provide job coaching and mentorship to PTI clients while they are applying for employment and transitioning to the workplace.
Provide emotional support to clients during the job application process, showing sensitivity to the needs of traumatized persons.
Help clients in job preparation through:
Identifying qualifications and eligibility of clients for employment search.
Assisting clients in the development of their resumes and application letters.
Assessing jobseekers' interests and preferences and match to market needs.
Conducting one-on-one sessions with jobseekers to determine and evaluate the types of jobs that they would want to apply.
Conducting coaching sessions and interview training designed to help overcome anxiety and build confidence for job interviews and employment screenings.
Provide follow-through sessions during job application and post-employment to understand client's progress and provide needed support.
Crisis intervention:
Prepare client crisis incident report, conduct risk assessment, develop and implement safety intervention plan
Conduct follow-up and monitoring according to crisis intervention protocol
Coordinate with client social worker, external service providers, and other PTI staff involved in client's safety intervention plan
Update follow-up reports of assigned clients
Project Implementation
Participate or assist in implementation of current and/or new project initiatives
Facilitate group activities which include but not limited to psycho-education sessions and outreach sessions
Conduct assessment and pre-referral interview for clients
Support program growth and development as necessary
Coordinate with Program Lead regarding task assignments and work schedules
Reporting and Documentation:
Prepare documentation report of all client engagement activities
Prepare program updates and reports.
Qualifications
Bachelor's degree in social work
Preferably a licensed social worker
Must have at least three (3) years of experience working with traumatized or highly marginalized or vulnerable populations
Must have at least two (2) years of experience in case management
High intellectual curiosity and strong willingness to continually learn and strengthen one's professional knowledge and skills
High levels of professionalism and a commitment to excellence
Ability to empathize and communicate effectively with people from different social backgrounds
Ability to build trusting relationships with clients from a variety of backgrounds
Ability to develop a genuine passion for the mission of PTI
Proficient in Microsoft Office applications
