General Responsibility
The Social Worker shall facilitate the planning, implementation, and evaluation of the student's social cases as prescribed by the Case Management policies of the organization.
Duties
1. Provides support to the supervision and implementation of appropriate social welfare programs and services provided by the organization to its clientele.
2. Acts as the Case Manager from client's admission to termination with the support of the allied services, performing to but not limited to the following:
· Gathers and validates information pertinent to the client's case.
· Prepares the case study report for Immediate Head's reviews, evaluation, and recommendations.
· Discusses the approved intervention program to client clearly and professionally.
· Facilitates implementation of approved intervention plans.
· Attends Case Conference as necessary as possible.
· Makes follow-ups as necessary or considered needed with client and concerned YF Employees.
· Submits progress (updates) reports to Immediate Head, as necessary as possible.
3. Makes referrals to suitable agencies or organizations (as needed) and establishes external linkages and networks necessary for the operations of the organization.
4. Attends to staff development plans per recommendation of Manager and Young Focus.
5. Assists the Immediate Head in planning, execution, and implementation of strategies for emergency response (i.e., fire, demolition).
6. Facilitates training needs assessment and Standard Guidelines for Case Management.
7. Facilitates approved life-skills activities and sessions to community, family, and students.
8. Acts as facilitator for group of people together for variety of purposes that may include community development or advocacies.
9. Contributes essential functions for family and community relations of the organization.
10. Performs other task that may be assigned from time to time.
