(PDS must be administered by any lawyer or barangay captain) ( plus 3 photocopies)
3. Certified True Copy (from School Registrar):
- 1 Diploma(+4 photocopies) and
- 2 Transcript of Records (TOR- plus 3 photocopies)
***both Pre-law & Law Diploma if LLB***
4. 2 Authenticated CSC Certificate of Eligibility (plus 3 photocopies)
5. Five (5)Photocopies of PRC Board Rating and valid PRC ID (for CPAs,Engrs,LET Passers, and others)
6. 5 Photocopies of BAR Rating and Certificate of Membership in the Philippine Bar (for lawyers)
7. Previous BIR Employees: Regional/National Office Clearance and Ombudsman Clearance
