• Open and close the hydrocollator tank before and after working hours.
• Call the Information Department for registration of out -patient PT procedures.
• Render the Physical Therapy procedures of in-patients and out-patients.
• Give PT home instructions to the patients and relatives.
• Write-in the physical therapy notes/ progress notes.
• Call the Information Department for post-charging of out-patient PT procedures.
• Print the out-patient requisition slip thru the Bizbox system.
• Inform the in-patient nurse-in-charge for PT procedure requisition.
• Write the patients information in the log-book and daily-census folder.
• Answer phone call inquiries or walk-in patient inquiries regarding Physiatrist appointment schedules, benefits of physical therapy and procedure cost.
• Keep the machines and the department safe and clean, otherwise report it to the PT Head
• Sign the Waste Segregation Monitoring sheet done by the House keeping.

• Inform the Linen department to collect the dirty laundry's.
• Receive the new washed laundry's from the Linen department.
• Help the PT Head on Weekly Census Report of patients and procedures.
• Inform the PT Head regarding out-patients who are not attending their regular PT schedules.
• Remind the out patients to write the department Survey Form.
• Attend the weekly Administrative Meeting during monday morning in case the PT head is not present.
• Check if office and hospital supplies are sufficient in the department.
• Remind the PT Head regarding insufficient office and hospital supplies.
• Help the PT Head collect the Post-Charge Slip for Physiatrist professional fee monthly report.

• Sign the PT Staff Schedules done by the PT Head.

• Help the PT Head with the Inter-department Annual Report of cases and procedures.
• Help the PT Head with the Inter-department Annual Report of income and expenses.
• Plan, prepare and carry out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Administer manual exercises, massage and/or traction to help relieve pain, increase the patient's strength, and decrease or prevent deformity and crippling.
• Instruct patient and family in treatment procedures to be continued at home.
• Confer with the patient, medical practitioners and appropriate other to plan, implement and assess the intervention program.
• Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.
• Record prognosis, treatment, response, and progress in patient's chart or enter information into computer.
• Obtain patient's informed consent to proposed interventions.
• Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
• Test and measure patient's strength, motor development and function, sensory perception, functional capacity, and respiratory and circulatory efficiency and record data.
• Identify and document goals, anticipated progress and plans for re-evaluation.
• Provide information to the patient about the proposed intervention, its material risks and expected benefits and any reasonable alternatives.
• Inform the patient when diagnosis reveals findings outside their scope and refer to an appropriate practitioner.
• Administer treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines.
• Teach physical therapy students as well as those in other health professions.
• Evaluate, fit, and adjust prosthetic and orthotic devices and recommend modification to orthotics.
• Conduct and support research and apply research findings to practice.
• Construct, maintain and repair medical supportive devices.
• Direct group rehabilitation activities.
• Provide educational information about physical therapy and physical therapists, injury, prevention, ergonomics and ways to promote health.
• Assist the health care personnel in their duties and in their needs as necessary.
• Conducts regular inspection of records, floor stocks supplies and equipment to determine and ensure completeness.
• Takes lead role in the provision of adequate and safe environment for patients and the personnel.
