Solid Ground International is a non-profit organization dedicated to the restoration of individuals exploited by human trafficking and gender-based violence. The mission of Solid Ground International is to provide housing, healing, and hope through transitional housing. We are seeking a full-time Registered Social Worker to provide case management support to our residents.

Duties and Responsibilities
Case Management
Establish, maintain, and update case management files for each resident on a regular basis
Complete social case study reports and intervention plans for all residents annually
Maintain a schedule of monthly check-in sessions with residents
Complete all necessary procedures and documents during the intake, discharge, and reintegration of each resident
Liaise with the referring agencies to complete formalities for the transfer of the resident to SGI
Schedule and attend case management conferences and keep minutes of the meeting
Complete reintegration follow-up procedures for each resident who successfully graduates from SGI
Complete home assessments for residents
Make referrals for residents needing professional mental health services
Assist the residents in finding stable housing and safe, legal work
Assist the residents in enrolling in educational institutions and in obtaining education related documents as needed
Organizational Support
Coordinate with partner agencies, educational institutions, health care providers, and employers concerning any issues that arise and plan for solutions with the resident
Cooperate with partner organizations to develop and implement care strategies to ensure each resident receives the necessary assistance in the continued healing of past trauma and the establishment of independent living
Assist in the teaching, implementation, and monitoring of life skills curriculum
Build and maintain relationships with key stakeholders, partner organizations, and government units
Comply with DSWD requirements regarding meetings, trainings, licensing, and accreditation
Cooperate with SGI staff in implementing proper intervention strategies to assist residents in their growth
Other Responsibilities
Complete regular reportorial requirements as established by the Director and Board of Directors
Comply with all requirements set forth by the Philippine Regulation Commission to maintain active registration
Participate in staff meetings, self-care plans, and mandatory group activities
Assist Program Coordinator/Director on ad hoc program management tasks
Qualifications
A registered Social Worker with the Professional Regulation Commission
Minimum of two (2) years of experience in case management with traumatized or marginalized populations
Excellence in verbal and written communication in English and Bisaya
Proficiency in Microsoft Office applications
Ability to effectively and compassionately communicate and cultivate trust with clients from diverse backgrounds
Adept at assessing and addressing client needs
Ability to build and maintain relationships with SGI staff, residents, partner organizations, and government stakeholders
Strong ability to maintain confidentiality and professional standards of conduct
