Job Summary
We are looking for a Project Manager to join our team for a Telehealth Program. As a Project Manager, you should be a quick learner with strong communication skills and have the ability to manage operations while constantly improving the processes.
Responsibilities:
Patient Engagement
Assess referred patient needs and assign them to specific providers based on criteria
Reach out to patients via phone, email, and SMS to secure an appointment for Palliative Care providers
Communicate program goals and manage patient expectations
Provide guidance and technical support for patients on how to connect to the platform
Optimize provider schedules by ensuring booked appointments will be completed
Answer inbound calls for patient inquiries, requests, and concerns
Facility Coordination
Connect with referring facilities or providers to secure patient’s medical records
Manage facility’s database and report referral progress to point persons
Coordinate for any additional information needed before the Palliative Care appointment
Send consult notes for all completed appointments
Operations and administrative tasks:
Plot all confirmed schedules and send meeting links to the patient
Create patient records and upload necessary documents to the platform
Keep accurate records of patient’s appointment status for reporting
Coordinate with the providers' post - consult to monitor patient appointment completion and secure consult notes
Collaborate with the project manager for process improvements and strategize on how to address any patient, provider, or committee concerns
Assist the nurse provider for patients that need an initial assessment
Supervise the Spanish-speaking appointment setter for timely achievement of goals and monitor the monthly work invoice based on performance
Skills/ Experience Required:
Proven appointment scheduling and customer service experience
Interpersonal communication skills
Outstanding listening skills and attention to detail
Task management
Excellent problem solving, critical thinking, and analytical skills.
Data Entry
Quick-learner and able to multitask
Minimum Requirement
Bachelors Degree Holder
Background in Healthcare, IT, and Customer Service Industry (is a plus)
Ideally with 2+ years of experience
Strong Written and Spoken English Communication Skills (Required)
Bilingual in Spanish is a plus but not required
Job Type: Full Time Permanent Work From Home
Salary: Php30,000/monthly
Required WFH Set-Up:
You should have a Fast & Reliable internet connection (minimum of 10mbps- preferably Fiber connection)
You should have your own Updated Desktop or Laptop with an excellent quality microphone, webcam, and headset. (not provided by the company)
