Job Description
RPM Alert management
Manage large amounts of outbound calls in a timely manner
Communicate with providers regarding acute and chronic patient conditions
Monthly wellness assessments
Chart-review
Medication reconciliation
Prescription refill requests
Care plan creation and management
Meet personal/team qualitative and quantitative targets
Other activities designed to close gaps in care
Chart/document all details/conversations in the CCM/RPM software
Qualifications
Active LPN/PVN/RN license
Passionate about patient care, possessing the ability to relate with empathy and compassion.
A passion for patient teaching is a must.
Quiet, HIPAA-Safe space to do your work
Experience in CCM/RPM Software (preferred)
Fluency in Spanish language (preferred)
Should have a strong internet connection (minimum of 20 MBPS)
Work location: Permanent Homebased
Work Hours: 4 days with 10 hours shift or 3 days with 13.5 hours shift (9am to 8/10pm Manila Time
