MUST BE BE PHARMACY MAJOR IN CLINICAL PHARMACY OR DOCTOR OF PHARMACY
WITH ATLEAST 1 YEAR WORK EXPERIENCE
• Conduct drug therapy assessment to determine whether the prescribed medicines are optimally meeting the patient's needs and goals of therapy.
• Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effect, and drug interactions, in performing their patient care activities in collaboration with other members of the health care team.
• Document the clinical pharmacy services provided through the different clinical pharmacy forms.
• Reports medication errors on weekly basis for review and assessment.
• Other task maybe assigned by the immediate supervisor
