Completely completed Personal Data Sheet (PDS) with recent image (CS Form No. 212, revised in 2017 and guided in the "Guide to completing the CSC PDS form" that can be downloaded from the CSC website;
Performance rating in the last rating period (if applicable);
Authenticated copy of the certificate of eligibility / rating / license;
Authenticated copy of the transcript of records / diploma;
Birth certificate / marriage contract (PSA); and6. Full local clearances (Barangay clearance, Mayor clearance, Police clearance, RTC court clearance, NBI clearance, and DI PNP clearance)
