All pertinent documents for ranking must be intriplicate packaged seperately in three folders. Applicant/ s may opt to submit their original documents orjust present the same upon the scheduled ranking interview. Any and all document/ s not submitted on orbefore the deadline will no longer be considered for assessment. In addition the following document/smust be included in the folders to be submitted.

Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212,Revised 2017) which can be downloaded at www.csc.gov.ph;
Performance rating in the present position for THREE (3) years/ rating period (if applicable);
Photocopy of certificate of eligibility/rating/license; and
Photocopy of Transcript of Records.
