Is currently looking for:
 
REGISTERED SOCIAL WORKER (RSW)
 
The RSW will be responsible for delivering social work services to inpatients, outpatients, and hospital staff. Key duties include providing discharge planning support, coordinating care to ensure continuity across all service areas, and addressing patient needs. The role also involves handling referrals from physicians, hospital personnel, patients, families, external agencies, and other healthcare professionals.
 
RESPONSIBILITIES:
Ensures that the Social Service Communication Form is accurately and completely filled out, and properly endorsed for approval.
Encodes patient details into the Hospital Information Management System (HIMS), verifies the accuracy and completeness of records, and ensures there are no duplicate patient identification numbers.
Assists during patient admissions by ensuring that all applicable forms are duly acknowledged and signed.
Registers and encodes charity patients in the HIMS, making sure they are properly documented.
Consistently delivers exceptional, customer-focused service to patients, families, and stakeholders.
Provides orientation to charity and package-deal patients regarding charity service processes, guidelines, and related procedures.
Assists in orienting patients and their families on hospital policies, discharge procedures, and available payment options.
Helps facilitate hospital tours for healthcare professionals, stakeholders, and educational groups.
Addresses concerns and inquiries from internal and external customers within the scope of responsibility, and promptly endorses matters beyond authority to the immediate superior.
QUALIFICATION:
Bachelor's Degree in Social Work.
Possess a valid license from the Professional Regulation Commission (PRC) as a Registered Social Worker.
