Education :
Completion of two years studies in college.
Training :
4 hours of relevant training.
Work Experience :
1 year of relevant experience
Competency :
Proficient in computer applications.

Documents:
1. Fully accomplished Personal Data Sheet (PDS) with recent passport size picture (CS Form No. 212,
Revised 2017), and with attached Work Experience Sheet, which may be downloaded at www.csc.gov.ph
2. Performance rating in the last rating period (if applicable);
3. Photocopy of certificate of eligibility/rating/license (for external applicants);
4. Photocopy of PSA or LCR Certificate of Live Birth (for external applicants);
5. Photocopy of scholastic/academic record such as diploma and Transcript of Records (for external
applicants);
6. Photocopy of valid NBI Clearance (for external applicants);
7. Drug test within the last 3 months or certification by the concerned Barangay Anti-Drug Abuse Council
(BADAC) on non-involvement in illegal drugs (for external applicants); and
8. Medical Certificate (CS Form No. 211, Revised 2018) issued by a licensed government physician (for
external applicants)
