JOB DESCRIPTION:
Receives endorsement from the outgoing shift.
Makes rounds with special attention to critical patients, new admissions, pre-operative, post-operative and patients with contraption.
Takes and records vital signs every four hours on all patients with specific orders, and for all critical ill patients and patients with deviant vital signs.
Joins doctor's rounds.
Makes rounds necessary for the purpose of observing, assessing and meeting patient's needs.
Makes necessary referral to attending physician.
Coordinates with different divisions/units regarding implementation of patient care.
Carries out, transcribes and communicates doctor's orders with other staff members if the orders have been changed or discontinued.
Checks charts and previous orders.
Times medications.
Changes and discards linens, serves bedpans, gives bed bath, perineal care, turns patients, etc.
Checks if the patient is prepared for scheduled procedures at X-ray, OR, etc.
Assists physicians in examination, treatment, dressing, etc.
Checks narcotics and records properly. Checks completeness of the E-cart and makes replacements as necessary.
Accomplishes discharge form and gives necessary instructions to patients/relatives.
Checks contraptions, IVF, etc. if properly functioning.
Records intake and output, empties output and cleans container.
Carries out admitting orders and notifies other services .department about admission.
Transports patient.
Receives post-operative patient and clarifies post-op orders when necessary.
Reviews and update Kardex, evaluates effectively nursing care done.
Assists patient during meals if unable to feed himself. Provides explanation to patient regarding nursing.
Attends and participates in staff meeting as called for by the administrator.
Abides by the hospital policy, rules and regulations and follow department's standard operating procedures.
Perform other related functions as may be assigned by a superior and Management.
QUALIFICATIONS
· Male or Female
· BSN Graduate, Registered Nurse
· PNA /ANSAP Member
· IV Therapy Card Holder/Preceptor
· Attended BLS/ACLS Training and other seminars and training related to nursing practice.
· With good oral and written communication skills.
