We are looking for a highly organized, professional, and compassionate Patient Care / Patient Success Coordinator to join our remote team. This role is ideal for someone with strong communication skills, excellent attention to detail, and experience supporting patients, clients, or healthcare teams in a fast-paced environment.
 
Role Overview
The Patient Care / Patient Success Coordinator will support the day-to-day coordination of patient care across Perennial's clinics and partner residences. You will be responsible for helping residents and families book appointments, coordinating with residence teams, supporting clinicians with scheduling and documentation workflows, and ensuring patients receive timely, professional, and compassionate support.
 
This is a non-clinical coordination role. You will not provide medical advice, make clinical decisions, or interpret medical results. Your role is to support communication, coordination, follow-up, and administrative workflows so that the clinical team can provide excellent care.
 
Key Responsibilities
Patient & Family Support
Respond to patient, resident, family, and residence inquiries by email and phone in a professional and compassionate manner.
Help schedule, reschedule, and confirm virtual and in-person appointments.
Follow up with residents, families, and residence teams regarding appointment details, required forms, and next steps.
Provide a high level of service and support to ensure patients and families feel cared for and informed.
Escalate urgent, sensitive, or clinical matters to the appropriate Perennial team member.
Clinic & Scheduling Coordination
Coordinate physician and nurse practitioner clinic schedules across multiple residences.
Prepare daily and weekly clinic lists for providers and residence teams.
Support virtual clinic scheduling, appointment reminders, and follow-up workflows.
Track appointment requests, cancellations, no-shows, and pending follow-ups.
Help ensure clinics run smoothly and that all parties have the information they need in advance.
EMR & Administrative Support
Enter and update patient information accurately in the EMR and internal tracking systems.
Upload, organize, and maintain documents, forms, and patient-related records.
Support patient onboarding, chart creation, appointment documentation workflows, and administrative follow-up.
Track outstanding tasks, pending requests, referrals, lab requisitions, forms, and other administrative items.
Maintain accurate records while following privacy and confidentiality requirements.
Residence & Team Communication
Communicate with retirement residence teams, including Directors of Care, Executive Directors, and care coordinators.
Help coordinate lists of residents who require appointments or follow-up.
Support internal communication between Perennial operations, clinicians, residences, and families.
Provide regular updates on outstanding items, clinic readiness, and patient coordination issues.
Identify workflow gaps and help improve processes as Perennial continues to grow.
Patient Success & Quality
Ensure patients and residence partners receive timely, clear, and professional communication.
Monitor follow-ups to make sure requests do not fall through the cracks.
Help improve the patient and residence experience by identifying recurring issues or delays.
Support new program rollouts, service launches, and operational improvements.
Represent Perennial with professionalism, empathy, and attention to detail.
Qualifications
Required
Excellent written and spoken English.
Prior experience in healthcare administration, patient coordination, customer support, medical virtual assistance, or a related role.
Strong attention to detail and ability to manage multiple tasks at once.
Comfortable speaking with patients, families, clinicians, and business partners.
Strong computer skills, including email, spreadsheets, online scheduling tools, and EMR/CRM systems.
Ability to work independently in a remote environment.
Professional, calm, and compassionate communication style.
Strong organizational skills and ability to follow structured processes.
Reliable internet connection, quiet workspace, and ability to work full-time Canadian business hours.
Nice to Have
Experience working with Canadian, U.S., or international healthcare clients.
Experience using an EMR, EHR, CRM, or patient management system.
Background as a medical virtual assistant, healthcare customer service representative, clinic administrator, nurse, or allied health administrator.
Experience supporting senior care, home care, retirement residences, or long-term care.
Familiarity with appointment scheduling, referrals, lab requisitions, prescription refill workflows, or clinical documentation support.
