Education :
Bachelor of Science in Nursing
Training :
4 hours of relevant training
Work Experience :
1 year of relevant experience
Competency :
Work effectively in vocational education and training; Participate in workplace communication; work in team
environment; Interact with customers; Implement quality standards and procedures; Promote programs and services; Provide heathcare services to students and personnel; Respond to emergency cases; Maintain health facilities; Maintain health records; Prepare reports

1. Duly Accomplished Personal Data Sheet with Experience Sheet (CSC Form 212, Revised 2017);
2. Service Records/Certificate of Employment;
3. List of training programs attended indicating the no. of training hours together with the certified photocopies of training certificates;
4. Performance Evaluation System (PES) Rating 2019 for outside applicants and Individual Performance Commitment Review (IPCR) Rating forJuly - December 2019 and January - June 2020 for TESDA applicants or applicants from other government agencies of at least a ' Very Satisfactory' Rating;
5. For government employee only: a copy of previous appointment;
6. Potential Assessment Forms to be accomplished by your Immediate Supervisor and one (1) Peer;
7. Statement of duties and responsibilities for the positions held indicating the specified period covered (mm-dd-yy to mm-dd-yy);
8. Certified true copy of transcript of records/diploma; and
9. Certified true copy of Eligibility by CSC or PRC
10. Applicants for TESD Specialist II, Instructor I to Assistant Professor II must be a holder of National TVET Trainers Certificate (NTTC)
