3. Certified true copy of transcript of Records (TOR);
4. Certified true copy of diploma and/ or certificate of graduation;
5. Certified true copy/ authenticated photocopy of certificate of eligibility/bar or board rating/license;
6. Photocopy of certificate of employment with actual duties and responsibilities (if applicable);
7. Photocopy of certificate of trainings/ seminars attented;
8. Performance rating in the last rating period (if applicable);
9. Certificate of Live Birth duly authenticated by the PSA or the LCR of the municipality or city where the birth was registered or recorded (for original and reemployment);
10. For married female employees, a Marriage Contract/ Certificate of duly authenticated by the PSA or the LCR of the municipality or city where the marriage was registered or recorded (for original and reemployment);
