JOB SUMMARY :
Responsible for conducting the screening of patient's qualification to avail medical assistance services and facilitation of the provisions of services to identified beneficiaries.
SPECIFIC JOB DUTIES :
1. Assesses indigent patient/clientele to possible referrals.
2. Makes referrals necessary in granting their request for assistance.
3. Makes social case study report that would serve as referrals to other government agencies.
4. Assists the Admitting Staff on duty, Billing section and Hospital Administrator with regards to the case of patient.
5. Conducts interview and data gathering related to patient's relatives.
6. Follow ups the credit and collection of all the Guaranty Letter from the government agency for the payment of Guaranty Letter
7. Assists the relatives of the patient to execute the promissory note and assesses all means and resources of the patient's relatives have been taken into considerations.
8. Monitors daily all patients experiencing difficulty in paying hospital bills for the prevention of Boarders.
9. Calls or informs the Doctors of the patient before discharged of the patient
10. Counsels the patient's family who has financial problem and having elective cases such as those suffering from heart, kidney and cancer disease and mental illness and trauma cases.
11. Coordinates with the billing, Credit and Collection Section to provide comprehensive assessment of the patient which includes emotional, psychological and economic assessment.
12. Conducts home visit as necessary.
13. Performs other duties that may be assigned from time to time.
IV. GENERAL JOB QUALIFICATIONS :
Education : Graduate of Bachelor of Science in Social Work.
Must be a Licensed Social Worker.
Experience/Training : Minimum of one (1) year relevant experience.
Knowledge/Skills/Attitude : Able to perform the functions of Social Worker
