Description
1. Assembles and prepares completeness of medical records of patients discharged from the hospital and compiles data from medical records for periodic statistical reports.
2. Assembles records of patients in established order for filing.
3. Review important and emergency room records to ensure that records are completely accomplished and signed by the attending physician or other professional staff and ensures receipt of all records of patients discharged from hospital by checking with the list of discharged patients based on daily census
4. Maintains follow up of records until data are completely accomplished
5. Computes and provide all statistical reports from records and other data.
6. Overall in-charge of filing and retrieval of medical records and updated index file of patient.
7. Maintains a sign out system of records and an updated index file of patients
8. Retrieves prior records for inclusion with current records when necessary
9. Files medical records by established numbering system and re-check files with proper retention system.
10. Responds to inquiries from hospital departments regarding concerns with patients' medical records.
11. Assist the PHIC clerk and Manager with other tasks related to patient records and PHIC claims when necessary.
12. Preparation of birth certificate
13. Preparation of mandatory reports required by DOH and PHIC
14. Performs other tasks which may be assigned from time to time
