General Description of the Job
· Working with physicians, nurses' supervisors and other clinical staff, as part of medical team focusing on patient outcomes, while maintaining cost. Experts in the therapeutic use of medications. Ensure the rational and effective use of medication.
II. Duties and Responsibilities
· Reviewing medical chart of the patients to determine appropriateness of medication therapy
· Evaluate patient's condition to ensure all issues are being treated
· Develop effective medication plans that minimize the risk of adverse side-effects
· Consult on dosages, medication substances and advice correct medication of drugs
· Collaborate with healthcare professionals to ensure optimal patient care
· Keep accurate documentation of medication plans and patient progress
· Responsible for promoting Antimicrobial Stewardship Program for the optimal use of antimicrobial agents, reducing the transmission of infections, and educating health care professionals, patients and the public
