JOB DESCRIPTION
Renders primary care to hemodialysis patients.
Assists in the admission of the patient into the unit and secures consent for series of Hemodialysis from the patient/ relative.
Does physical assessment before, during and after hemodialysis.
Weighs patient and checks vital signs before and after hemodialysis. Documents and informs attending nephrologists of patient’s condition.
Assesses, cleans, dresses and declots (when necessary) access sites. Prepares and cannulates AV fistula and Gortex aseptically before hooking patient to Hemodialysis machines.
Hooks and unhooks responsibly all patients from hemodialysis machines.
Checks the whole treatment set-up including dialyzer for any blood leakage and other technical problem which may need immediate action.
Monitors hemodynamics and records patient’s blood pressure and heart rate every 30 minutes and as often as necessary for unstable patients.
Checks dialysate conductivity, temperature, heparin infusion pump function. Records and adjusts blood flow rate, TMP, venous and arterial pressure.
Collects blood specimen for routine laboratory tests.
Administers medication and treatment to usual side effects of Hemodialysis according to unit’s standard operating procedures.
Performs blood transfusion and institutes emergency responses to any transfusion reaction.
Assists nephrologists when examining patient and in times of emergency situation like “CODE BLUE”.
Prepares the equipment and assists vascular surgeon during bedside insertion of subclavian catheter.
Provides adequate post hemodialysis care including completion of patient’s chart.
Supervises the nursing orderly with regards to their function contributory to patient’s total care such as proper technique of handling and packing instruments, sterilization of supplies, disinfection of machines and dialyzer and keeps the unit clean and order.
Coordinates with other medical health team such as laboratory, nuclear laboratory, X-Ray, ultrasound, etc.
Provides patient and relative educational programs such as lectures on diet, fluid intake, anemia, erythropoietin, lifestyle and disease process and value adhering to their regular schedule.
18. Checks unit stocks, supplies and charges items used by the patients.
19. Participates in the general activities of Nursing Service.
20. Completes patient chart after treatment including laboratory flow sheet and hemodialysis flow sheet.
21. Provides assistance to patient who will transfer and travel to other dialysis centers by coordination and completion of endorsement form.
22. Helps hemodialysis technician in rendering post-mortem care to patient that expired in the unit and offer comfort to the bereaved family.
23. In the absence of the Head Nurse, takes supervision of all other staffs and personnel.
24. Performs other duties as deemed necessary / as directed
