Our Client
In this position, you will be assigned to our client, an e-Health SaaS company based in Melbourne, Australia, supporting respiratory health Management. Their world-first technology detects wheeze, a typical symptom of asthma, COPD, and respiratory disease to provide an objective measure of airway limitation.
Your Responsibilities
Support and lead the delivery of Remote Asthma Management Program (RAMP)
Provide medical device education
Program onboarding
4-weekly 1:1 patient consultations and off-boarding of patients recruited by their treating physician
Work collaboratively with Program Lead and other internal stakeholders whilst forming strong relationships with treating GP/Physicians
Contact patients as per program schedule and/or as required, either via video, telephone to provide ongoing program support
Review data received from patients as they complete their recordings and overall engagement in the Respiri Asthma Management Program (RAMP)
Use data received, along with answers to quality-of-life questionnaires to produce monthly updates and reports for the patients
Provide patients with high-quality care and advice based on best practice guidelines and data in the reports
Assist patients with goal setting and progress review, highlighting areas for improvement in a supportive and encouraging way, emphasizing positive reinforcement
Escalate any major and urgent issues to the Program Lead, the patients' GP/Physician/Emergency Services as necessary
Contribute to improving services and products by proactively providing feedback to the Program Lead
Maintain any clinical records/systems/documents as required and in line with legislation and guidelines
Support materials development, ideas, guidance, review, and approval
Your Background
Registered Nurse experienced in Asthma Education is highly regarded
Experience in managing clinical programs and/or telehealth medicine
Must have significant experience in health and health systems, preferably Cliniko or similar
Experience in providing education and ongoing patient support in a remote setting
Willingness and flexibility to do admin and reporting tasks
High analytical skills with proven ability to capture patient feedback and produce clinical reports
Experience in providing remote support and care coordination support to patients
Demonstrated experience of working collaboratively within a team, but also autonomously
An excellent track record of making things happen and achieving outcomes
High level interpersonal, communication, consultation, and negotiation skills with particular emphasis on demonstrated capacity to work with internal and external stakeholders to achieve successful outcomes
Experience or comfort to work remotely and collaborating via shared environments/project management software is highly regarded (e.g. Monday.com, Trello)
