1.4.5 Corporate Flight Safety Officer (CFSO)
1.4.5.1 The CFSO reports directly to the Corporate Flight Safety Manager (CFSM)
1.4.5.2 Analyze and perform FDA Data after the technical analysis
1.4.5.3 Regularly review safety data collection and processing system (SDCPS) known as AirFase, analyze risk related to flight safety and report to the CFSM, HCS and the CSD team as required
1.4.5.4 Create reports or be part of report creation required for flight safety occurrence investigation report (OIR)
1.4.5.5 Participate in safety meetings (SAG, SRB, etc)
1.4.5.6 Facilitate and promote CSD activities
1.4.5.7 To recommend or propose training to continuously improve capabilities to HCS, CFSM, and CSD team
1.4.5.8 Coordinate with flight operation training and standard and maintenance engineering on the trend, significant event related to aircraft and technical operations
1.4.5.9 Conduct research and development on safety related matters
1.4.5.10 Responsible for the interpretation for analysis of FDA with respect to the technical aspects of aircraft operations
1.4.5.11 Participate in the preparation of safety audit, HIRA. formulation of flight related matters and the conduct of station observation and assessment.
1.4.5.12 Assist HCS or CFSM de-briefing and counselling of pilots
1.4.5.13 Perform risk assessment and close safety reports
1.4.5.14 Assist HCS or CFSM in creating safety notice and safety promotion materials as deemed necessary
1.4.5.15 Prepare the minute of meeting and its distribution
1.4.5.16 Coordination of pilot volunteer program
1.4.5.17 Assist in investigation of flight incident and/or accident with the aim of prevention and/or avoiding recurrence
1.4.5.18 Assist conduct of technical and non-technical investigation/inquiry of any flight safety related events , as and when required
1.4.5.19 Support, follow up on feedback and CAP from respective person/department responsible
1.4.5.20 Attend the departmental, regional, and SRB meetings regularly and flight safety SAG 1.4.5.21 Under no circumstances, disclose any confidential, sensitive or departmental information to anyone inside and outside the company without the consent of the AM and/or HCS (e.g. personnel information, safety investigation reports etc)
1.4.5.22 Assist in the creation of parameters for measurement of safety performance
1.4.5.23 Assist in safety survey
1.4.5.24 Conduct non-technical investigation of any aviation safety related event, when required 1.4.5.25 Participate in the preparation of safety audit. HIRA formulation of flight related matters and the conduct of station observation and assessment

2.1.5 Flight Safety Officer (FSO)
2.1.5.1 The Flight Safety Officer is usually assigned by the Flight Safety Manager to conduct discussion with flight crew involved in events highlighted by the FSDAP. This position requires good people skills and a positive attitude towards safety education. Also, the Flight Crew Contact Person Is permitted to connect identifying data with the event as he / she would require the trust of flight crew and management for upholding Integrity and good judgment. This role Is help by a member of the Flight Safety Team.
2.1.5.2 The FSO are pilots that reports directly to the CFSM and has duties and responsibilities as stated in this section. The FSO could also be an FODC observer once selected and trained with a line operations monitoring program.
a. Obtain follow-up information from the flight crew member (i.e. line pilots) involved in a particular event.
b. Conduct flight data analysis and investigation of events, including the conduction of constructive counseling sessions for pilots.
c. Collect any relevant information (weather data, pilot report and inputs) in order to understand particular events.
d. Classify and assess criticality of an occurrence and risk tolerability of a hazards reported through ASR and/or OHIR.
e. Identify the flight operations safety Issues through major occurrences. statistics and trend analysis.
f. Hazard Identification and Risk Assessment (HIRA) and Evaluation
i. Ensure the HIRA are carried out for:
1. Flight safety reports received
2. Flight incidents and accidents
3. FDA triggered events
4. Any flight safety event or occurrence required
ii. Safety Investigations
1. In assistance to CFSM, investigate the flight accidents or incidents involving company proper ties or personnel, as and when required, with the aim of prevention and/or avoiding recurrence.
iii. Safety Audits
1. In assistance to CFSM, perform the periodic operational safety audits to ensure safety standards are being maintained and continually exercised.
iv. Others Duties and Responsibilities
1. Attend the SRB, SAG and SMS meetings regularly.
2. Participate in interviewing and promotional exercises for pilots.
3. Under no circumstances, disclose any confidential, sensitive or departmental information to anyone inside or outside the company without the consent of the AM (e.g. personnel information, safety investigation reports, etc.).
g. Technical support for personnel responsible for ensuring the serviceability of mandatory FDA systems and equipment.
