Primary Function:
Responsible in administering medical services and implementing health and safety programs in the company
Duties:
1. Performs First Aid Treatment
- Burns
- Laceration
- Crushing Injuries
- Fractured
- Minor Medical Ailment
2. Assists in organizing Annual Physical Examination for Regular Employees
3. Conducts Physical Health Assessment to Applicants
4. Processes the following procedures:
- Fit to Work
- Back- to- Work
- Undertime and Cut-time due to medical condition
- Sick leave
- Maternity and Paternity Leave
5. Encodes employees' leaves in the system
6. Issues referral slip for medical assessment to employees for regularization
7. Examines and attends to medical concerns of all employees
8. Requests and replenishes medical supplies
9. Prepares the following documents and reports:
- Monthly Medical Info
- MRF for Nurse Aides
- SSS/ EC Sickness Notification
- EC Medical Reimbursement
- SSS Sickness Half Sheet
- Accident Logbook EC Monitoring
- Paternity Notification
- Request for Additional Comp
- Monthly Medical/ Dental Transmittal
- Justification for Mimeo of Forms
