DUTIES AND RESPONSIBILITIES:
1. Participates in ward and clinical pharmacy services and provides advice to ensure safe and cost effective prescribing and drug expenditure control.
2. Undertakes drug history taking, where possible, of all patients admitted to the medical center to ensure continuity of their medication and record relevant information in order to facilitate the discharge process. This involves communication with the patient, their prescribing physician and hospital medical staff and reference to the patient's notes in order to obtain accurate details of medication, allergies and other relevant clinical information. This will involve checking of the patient's own medication ( if previously taken by patient) and assessing it's suitability for use where appropriate
3. Monitor's use and administration of patient's drug regimens and appropriateness of serum drug levels; verifies appropriateness of dose; provides baseline for adjustment; and consults with physicians and nursing as to the appropriateness of drug use; reviews drugs and recommends additions to formulary. Likewise, assesses the patient's response to therapy, interprets blood results and depending on their clinical parameters, either adjusts drug doses in accordance with local procedures or advises the clinician appropriately.
4. Monitors and evaluates adverse drug reactions and pharmacist interventions; compiles and reports statistical information; develops quality assurance plans top assess appropriate use of drugs.
5. Coordinates clinical pharmacy activities with physicians and nursing, pharmacy and laboratory personnel; develops and assists in the implementation of clinical pathways and therapeutic guidelines. Participates on consultant ward rounds where applicable.
6. Records clinical interventions and activities in line with local hospital policy.
QUALIFICATIONS:
Educational qualifications:
Must possess a Diploma in BSc In Pharmacy, with at least 3 years of previous hospital experience, and at least 1 year specializing in the area of Clinical Pharmacy.
The Doctor of Pharmacy (PharmD)
Familiar with AMS.
Must be holder of a valid Philippine License to practice Pharmacy as issued by the Professional Regulation Commission (PRC) Board of Pharmacy.
Must posses at least 12 months pre-registration training and experience in Clinical Pharmacy or any related specialty training programs.
