Salary: 25,000 - 30,000 Peso/a month
Reports To: Client, Account Manager
Primary Relationships: Client Team

Responsibilities:
Work collaboratively with the Clinical team, documenting assessment, case notes, care coordination, and utilization management within the EHR system.
Work with the participants, their legal representatives, circles of support, and primary care physicians to:
Develop goals associated with the participants assessed needs and individual circumstances
Identify resources through friends, family, and/or publicly funded program
Educate, counsel, and advocate to and on behalf of participants
Assist with care coordination and identify service providers and community resources to help assure the timely, effective, and efficient mobilization and allocation of the services
Monthly/Bimonthly case management calls - by maintaining ongoing contact with participants - including monthly telephone calls to monitor for changes in health, mood, social integration, functionality, and overall well-being
In conjunction with physicians, evaluates and develops baseline medical and psychosocial evaluations and individualized patient care/treatment plans. * Encourages members to follow prescribed courses of care (e.g., drug therapy, physical therapy).
Consults with internal and external physicians, health care providers, discharge planning and outside agencies regarding continued care/treatment, hospitalization or referral to support services or placement.
Develops and collects data; trends utilization of health care resources.
Requirements:
Bachelor's Degree in Social Work
1 year of relevant experience
1 year experience in project development and management particularly on community-driven development programs, the conduct of case management and position/s involving management and supervision.
