Job Summary:
 
The PTI Aftercare Program Assistant is primarily responsible for providing coordinative and administrative support to different activities and initiatives implemented by the Aftercare team. The role entails data management for client cases within the team. Secondarily, the Aftercare Program Assistant will also handle some case management tasks, and work directly with clients. Critical to role is the ability to effectively manage multiple tasks, to have high level planning and organizing skills, to have high inter-personal skills, and the ability to develop a trusting relationship with clients.
 
Duties and Responsibilities:
 
Program Support
Provide support for programming, activities, and events initiated by the Aftercare Team
Prepare logistics for program implementation including preparation of materials, venue and other vital items for each specific activity
Coordinate with Aftercare Program Director, respective Program Lead and Aftercare Counselors/Social Worker in planning events and activities
Coordinate with internal and external partners in implementing different activities, if needed
Create and disseminates program communications and posts program information in PTI social media platforms, particularly in PTI FB groups for clients
Monitor project implementation is adherent to policies and objectives of the organization
Case management:
Facilitate client intake procedures and assess client needs 
Develop, implement, monitor, and evaluate client case plans and progress
Conduct sessions using motivational interviewing with clients to facilitate goal development activities
Provide job coaching and mentorship to PTI clients while they are applying for employment and transitioning to the workplace
Facilitate access to resources to help the clients in their job application process and transitioning to the workplace
Facilitate case coordination and information sharing with client social workers and other service providers involved
Process client referrals and applications to PTI sub-programs (education, work immersion, social services)
Coordinate with assigned PTI sub-program staff (education, work immersion, social services) 
Prepare client crisis incident report, conduct risk assessment, develop and implement safety intervention plan
Conduct follow-up and monitoring according to crisis intervention protocol
Monitor client's progress and determine case closure following standard protocols
Maintain client records and progress notes
Data Management
Organize data collection based on guidance from Aftercare Director and in accordance with organizational policies and guidelines
Support the collection and analysis of quantitative data for reports
Provide data entry support services for Aftercare projects
Coordinate with other Aftercare staff in collecting data and providing support to staff regarding data reporting, if needed.
Coordinate the collection and analysis of data for reporting
Maintain the accuracy, integrity, and security of computerized records systems.
Contribute to data quality and integrity by reviewing and verifying data
Manage program database
Reporting and Documentation:
Prepare documentation report of all client engagement activities
Prepare program updates and reports.
Qualifications
Bachelor's degree in social work
Must have at least one (1) to two (2) years of experience working with traumatized or highly marginalized or vulnerable populations
High intellectual curiosity and strong willingness to continually learn and strengthen one's professional knowledge and skills
High levels of professionalism and a commitment to excellence
Ability to empathize and communicate effectively with people from different social backgrounds
Ability to build trusting relationships with clients from a variety of backgrounds
Ability to develop a genuine passion for the mission of PTI
Proficient in Microsoft Office applications
