3. Photocopy of Birth Certificate (PSA)
4. Certified photocopy of Performance rating in the last rating period, must be Very Satisfactory (VS) (if applicable).
5. Photocopy of Certificate of Eligibility/rating/license
6. Photocopy of PRC license and Board Rating (if applicable)
7. Photocopy of Transcript of Records and Diploma.
8. Photocopy of Certificates of Training programs completed.
9. Certificate of Employment from all previous employers or Service Record (for government employees from other agencies). By submitting these documents/records, you agree that your information will be used to process your application.
