4. Original Copy of the authenticated certificate of eligibility/ report of rating/valid license issued by CSC,
LTO, PRC, Supreme Court, MARINA, etc; and
5. Original copy of the certified true copies of Transcript of Records (TOR) and diploma from the issuing
school .
